Vaccine Exemption for Medical Declinations identified as contraindications and precautions by the Centers for Disease Control and Prevention (CDC).
Step 1: Verify that your medical condition is identified by the CDC as a contraindication or precaution for the vaccine you wish to decline.

Step 2: Complete the declination form
Please provide a clear and specific statement outlining your reason for declining the vaccine.
Step 3: Obtain supporting documentation from a licensed practitioner for your medical condition.
Document must be on official letterhead and clearly state which vaccine you are unable to receive, verifying the particular contraindication(s) relied upon.
Step 4: Upload documents (declination, supporting documentation) to SentryMD.
Wellness 360 will review documents and contact you to confirm the status of your declination, whether it was approved or if additional information is needed.
Vaccine exemptions for medical conditions not identified as contraindications or precautions by the CDC may be requested as a reasonable accommodation under the Americans with Disabilities Act (ADA) if the individual has a qualifying disability.

Step 2: Notify your Human Resources partner regarding your request by calling (210) 567-8847 or email hrpartners@uthscsa.edu

Step 3: If the accommodation is approved, it must be resubmitted annually.
