Flu Shot Services - Wellness 360

Flu Vaccine Information:

Please review the instructions below before attending a flu clinic to receive your flu shot. Some clinics are only open to certain groups at UT Health San Antonio. If you attend a clinic not intended for you, you may not receive a flu shot.

Employees who obtain their flu immunization from a site other than Wellness 360 should send their vaccine record to EHWC@uthscsa.edu to have their employee information updated.

Download and complete your consent form from the purple box above.

Please bring a photocopy of your insurance card (front and back), showing name and date of birth of the primary card holder. If you are not the primary on the insurance card, write the name and date of birth of the primary on the photocopy of the insurance card.

Attach a photocopy of your insurance card front and back to your consent form. Note: We accept BCBS, Medicare, and all PPOs EXCEPT CIGNA. We do not accept HMOs and Tricare Standard.

For Clinical/Patient-Facing Employees Only:

If you choose to decline the flu vaccine for the season, you are required to complete and submit the attached declination form (pdf), providing a clear and specific reason for your decision. Submit all documentation through your SentryMD account.
Important Notes:

Medical Exemptions: If you are declining for medical reasons, you must also provide a letter from your healthcare provider. This letter must be on official letterhead and clearly state which vaccines you are unable to receive and the medical reason(s) for the exemption.
If you decline the flu vaccine, you will be required to wear a mask throughout the duration of flu season.
A new declination form must be submitted each flu season.
